propose des r\u00e9ponses p\u00e9dagogiques aux besoins \u00e9ducatifs particuliers des \u00e9l\u00e8ves, et ceci, avant la mise en \u0153uvre ou dans le cadre de la mise en \u0153uvre d\u2019un programme personnalis\u00e9 de r\u00e9ussite \u00e9ducative (PPRE), d\u2019un plan d\u2019accompagnement personnalis\u00e9 (PAP), d\u2019un projet d\u2019accueil individualis\u00e9 (PAI) ou d\u2019un projet personnalis\u00e9 de scolarisation (PPS).`<\/p>\n<p>Con\u00e7u pour tous les professionnels qui accompagnent l\u2019\u00e9l\u00e8ve dans sa scolarit\u00e9 (professeurs et chefs d\u2019\u00e9tablissement, m\u00e9decins de l\u2019\u00e9ducation nationale, professionnels des MDPH\u2026) le livret de parcours inclusif est une application num\u00e9rique accessible, depuis un ordinateur, qui vise \u00e0 faciliter :<\/p>\n<ul>\n<li><strong>la mise en place rapide et effective des am\u00e9nagements et adaptations<\/strong>, d\u00e8s l\u2019identification d\u2019un besoin \u00e9ducatif particulier par l\u2019enseignant, qui pourra s\u2019appuyer sur une banque de donn\u00e9es d\u2019am\u00e9nagements et d\u2019adaptations mobilisables tout au long du parcours de l\u2019\u00e9l\u00e8ve en fonction de ses besoins ;<\/li>\n<li><strong>la simplification des proc\u00e9dures de renseignement et d&rsquo;\u00e9dition des plans et projets<\/strong> par l\u2019\u00e9quipe p\u00e9dagogique (<a class=\"media-document\" href=\"https:\/\/eduscol.education.fr\/document\/21427\/download\" target=\"_blank\" rel=\"noopener\" data-ati=\"\" data-ati-name=\"ecole_inclusive_dossier_extrait_QPPQ_376117_378410.pdf|21427\" data-ati-chapter-1=\"document\" data-ati-type=\"download\">programme personnalis\u00e9 de r\u00e9ussite \u00e9ducative &#8211; PPRE -, plan d\u2019accompagnement personnalis\u00e9 &#8211; PAP -, guide d&rsquo;\u00e9valuation des besoins de compensation en mati\u00e8re de scolarisation (GEVA-Sco premi\u00e8re demande) document de mise en \u0153uvre du projet personnalis\u00e9 de scolarisation &#8211; PPS<\/a>)<\/li>\n<li><strong>la formalisation de ces diff\u00e9rents plans et projets<\/strong> permettant d\u2019organiser, en tant que de besoin, le recours \u00e0 divers am\u00e9nagements en association avec les familles ;<\/li>\n<li><strong>la circulation d\u2019information <\/strong>entre
